APPOINTMENT, PAYMENT, AND CONFIDENTIALITY POLICIES
Appointment and Payment Policies
The day and time we have selected to meet will be reserved only for you each week unless other arrangements are made. Because this is your time and the progress of therapy is best served with consistency and commitment, I will be available and ready for your appointment. It is important that you also make this commitment. A minimum of 24 hours notice is required if you must cancel or reschedule. Without exception, your full fee will be charged for missed appointments without the minimum 24 hour notice. Fees are due at the time of your appointment unless other arrangements have been made in advance. Some insurance companies may reimburse for services. If your insurance covers some portion of your fee, and in agreement with you. I may bill them directly. In these cases the co-pay or deductible is due at the time of service unless previous arrangements have been made. If you are paying a reduced fee, please notify me of changes in job or other financial circumstances so we can reassess. If necessary, every attempt will be made to make payment arrangements with you. However, please note, unless prior arrangements have been made, any fees left outstanding after 90 days following the termination of treatment will be sent to my collection agency. There is a bank service fee for checks returned from the bank. I will notify you if this occurs. Payment is due at your next appointment along with your regular fee for that session. Termination session: I request that your last appointment with me be in my office, not by phone. This allows time for review of the work you have done in therapy and closure between us. Confidentiality Policy All information disclosed within session is confidential and may not be revealed to anyone without your written permission except where disclosure is required by law. Disclosure may be required in the following circumstances: Where there is reasonable suspicion that you present a danger to yourself or others unless protective measures are taken and where there is information that would facilitate treatment in a medical emergency. I am required to advise the appropriate authorities of any suspected child or elderly abuse.
